A Program That Puts You in Charge of Your Care

The Consumer Directed Personal Assistance Program (CDPAP) is a self-directed care program that gives qualifying New Yorkers the freedom to choose your own caregiver, someone you trust and who understands your needs. Public Partnerships LLC (PPL),  the statewide fiscal intermediary for CDPAP, helps you understand how CDPAP works, and we support you in directing your care if you qualify and decide CDPAP is right for you.

Is CDPAP Right for You?

If you’re Medicaid-eligible and in need of home care,
CDPAP might be right for you.

You May Qualify If You:

  • Are already receiving Medicaid benefits
  • Have a need for home care services, including assistance with daily living activities or skilled nursing
  • Have received authorization from your local department of social services or managed care plan
  • Can self-direct your care (or have a designated representative who can help)
  • Want to hire your own personal caregiver, or "personal assistant"

What You Need To Do Next:

  1. Confirm Your Medicaid & Home Care Authorization
    Make sure you’re approved for home care services through your managed care plan or local department of social services.
  2. Get Authorized for CDPAP
    Have your managed care plan or local department of social services initiate the process to register for CDPAP. 
  3. Learn How CDPAP Works
    We’ll help you understand how the program works. If you qualify and decide it is the right choice for you, we’ll help you register and provide ongoing support.
  4. Choose Your Caregiver
    Pick someone you trust — family, a friend, or another person you choose (spouses and legal guardians of children younger than 21 are not eligible under NY rules).
  5. Get Support From PPL
    We walk you through registration steps and provide ongoing guidance so directing your care feels simple.

What is the Difference between CDPAP and PCS?

CDPAP is designed to give you real control, flexibility, and support in your home care. With PPL as your partner, you get even more clarity and confidence throughout your journey.

My caregiver can be a close personal relative, including a parent (of an adult consumer), son, daughter, son-in-law, or daughter-in-law.

PPL works with many partners throughout New York State to provide support in the ways that are most helpful to consumers and personal assistants. These partners are called CDPAP facilitators, and they have specialized skills to assist people with different needs and in a wide variety of languages.

Who Can Be a Caregiver?

Choose Someone Who Knows You Best

With CDPAP, you pick the caregiver who fits your needs, whether it’s a family member, friend, or someone else you trust (excluding spouses or parents/legal guardians of individuals younger than 21).

CDPAP Personal Assistant (Caregiver) Requirements:

  • Must be at least 18 years old.
  • Must have a valid Social Security Number.
  • Must be allowed to work in the U.S.
  • Cannot be a parent or a guardian of a consumer who is under the age of 21.
  • Cannot be the consumer’s spouse.
  • Cannot be the consumer’s designated representative.
